Suspected Wet AMD
L&D AMD Pathway – updated dedicated email box for all Wet AMD referrals requiring a fast-track appointment
For patients presenting at local Optometrist practices who may be suspected to have “wet AMD,” a rapid access referral form needs to be completed (Wet AMD Form Link)and emailed to ldh-tr.AMDcoordinator@nhs.net The patient should hear from the AMD co-ordinator within 2 weeks of receipt of referral with an appointment. It is good practice to explain this to the patient and ask them to contact you if they have not heard within this time. In these circumstances, please contact the AMD co-ordinator on 01582 497328 to confirm they have received the referral.
For all practice managers and ophthalmic directors, please ensure the above pathway is made clear to all of your Optometrists (locum or resident). Please remind them that referrals for suspect Wet AMD should NOT be emailed to the general ophthalmology referral email box or acute email box.
